Certificate of Origin Guidelines |

1. The Greater Omaha Chamber of Commerce will only seal completed COOs. The requesting member will be required to complete and sign the certificate before it is submitted to the Omaha Chamber, effective immediately. An electronic copy of the certificate can be accessed here.
2. The Omaha Chamber will only seal COOs for member companies. We do not seal COOs for non-members.
3. All COOs must include a reference number provided by the Omaha Chamber. Members can call the Omaha Chamber to request a pre-assigned range of reference numbers or an individual number. The Omaha Chamber will maintain a file of reference numbers.
4. An indemnification agreement must be submitted with each COO request. An electronic copy of the agreement will be provided to members upon request. Each agreement must include the COO reference number, and must be printed on the member company’s letterhead. A copy of the indemnification agreement can be accessed by clicking here.
5. Each member who submits a COO to the Omaha Chamber must include corresponding manufacturer’s invoices, shipper’s affidavits, and other documents that confirm the origin of the goods. Copies of these documents, including the COO, will be kept by the Omaha Chamber for a period of three years.
6. The Omaha Chamber will charge $10 per certificate sealed for the first 100 certificates. Each additional certificate will be $5. These totals are for the calendar year, not per request.
7. The Greater Omaha Chamber of Commerce will make every effort to process your COO request in a timely manner. Because sealing depends on staffing availability, members are urged to submit COOs prior to the day of shipment when possible. |
